Field Care Manager: Community LTSS Navigator
Molina Healthcare of Illinois
Molina Healthcare is seeking a Care Coordinator to support care management and LTSS activities for high-need members. You will collaborate with a multidisciplinary team to implement care plans, monitor progress, and ensure access to needed services across the continuum.
