Remote Call Post-Acute Nurse Practitioner

TeamHealth is seeking a quality-driven nurse practitioner to join our call solutions remote telephonic clinical call coverage team for weekend day coverage. Full-time and part-time opportunities are available, providing weekend day clinical telephonic call coverage for skilled nursing facilities, with excellent support and training, and autonomy. Compensation is estimated range of $60 - $60 hourly. In this role, you'll provide high-quality telephonic care to a diverse patient population, collaborating with an experienced multidisciplinary team. You will have an opportunity to deliver exceptional care during life's pivotal moments. TeamHealth's innovative population health data reports will guide you towards optimal and timely care for our patient population. Key Responsibilities Triage, implement, and adjust individualized care plans based on patient needs Coordinate care to optimize and improve patient outcomes Collaborate with nursing staff, physicians, and other healthcare professionals in a team approach Maintain accurate, timely documentation in compliance with regulations Qualifications Multiple state NP licenses are required; candidates with active New York and Maryland NP licenses are strongly preferred Ability to obtain and maintain additional state licenses as needed Experience in post-acute and/or telephone triage required Strong clinical foundation and knowledge of healthcare regulatory standards Excellent communication and interpersonal skills Collaborative, team-oriented approach with a positive outlook Weekend availability required Why Join TeamHealth?
