Job Responsibilities Evaluate, diagnose, and treat patients with physical impairments in a post-acute setting Develop and implement individualized treatment plans based on patient needs and clinical findings Collaborate with interdisciplinary teams to ensure coordinated patient care and recovery planning Supervise and work closely with Physical Therapist Assistants as neededMonitor patient progress and modify treatment plans based on clinical outcomes Educate patients and caregivers on mobility, safety, and home exercise programs Maintain accurate and timely documentation in compliance with facility and regulatory standards Requirements Active state license as a Physical Therapist in Tennessee Minimum of 1 year of recent PT experience preferred, ideally in post-acute, SNF, or rehabilitation settings Strong clinical reasoning and patient care skills Ability to work collaboratively within a multidisciplinary healthcare team Full major medical, dental, and vision benefits Supportive and collaborative clinical environment Opportunity to make a meaningful difference in patient recovery and quality of life #J-18808-Ljbffr
