You will work closely with other therapists, support staff, and healthcare providers in creating and executing effective treatment plans. Our team-centric culture encourages knowledge sharing and professional growth.Key ResponsibilitiesDevelop individualized rehabilitation plans that address the unique needs of each patient based on assessments and consultations with physicians.Oversee and mentor physical therapy assistants to ensure they appropriately implement prescribed therapeutic activities and interventions.Conduct thorough evaluations of patients' mobility and function using diagnostic and prognostic testing methods.Assess patients' home environments and recommend adjustments to improve their independence and safety during rehabilitation.Administer personalized therapeutic exercises, both manual and non-manual, while guiding patients to engage in functional activities.Utilize a variety of treatment modalities, including traction, massage, and physical agent applications, to promote healing and recovery.Fit and adjust prosthetic and orthotic devices, working in conjunction with physicians for modifications to enhance patient outcomes.Maintain accurate and comprehensive clinical records, including patient evaluations, care plans, and progress notes, ensuring compliance with the OASIS system.Participate in interdisciplinary team meetings to discuss patient progress and refine care plans collaboratively.Communicate effectively with patients, families, and other healthcare professionals to ensure clarity in treatment processes and goals.Provide patient education on the use and care of therapy equipment as well as on therapeutic exercises.Assist with functional assessments, resource utilization assessments, and the ongoing revision of therapy plans.Timely submission of clinical and administrative documentation in line with company policies.Patient Population / Client FocusOur primary focus includes a diverse patient population ranging from individuals recovering from surgery and sports injuries to elderly clients aiming to maintain their independence through physical rehabilitation. We strive to provide personalized care that addresses each patient's physical, emotional, and social needs, ensuring comprehensive therapeutic support throughout their recovery journey.QualificationsA Doctorate of Physical Therapy (DPT) from an accredited institution.Current state licensure as a Doctor of Physical Therapy (DPT) in good standing.A capacity for creative problem-solving and critical thinking in clinical situations.Excellent communication skills, both verbal and written.A collaborative approach towards patient care and teamwork.Preferred ExperienceAt least one year of direct experience as a Physical Therapist in acute care or home care settings.Proven ability to work effectively within a multidisciplinary team environment.Experience with electronic health records and telehealth methodologies is a plus.Compensation and BenefitsThis role offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
