St. Vincent De Paul

St. Vincent de Paul seeks an IHC Case Manager at Ozanam Manor in Phoenix to support residents of shelter and transitional housing with trauma-informed, strengths-based case management. You’ll help residents access services, advocate for needs, and remove barriers to permanent housing while promoting SVdP values of respect and dignity. Responsibilities include weekly resident meetings, individualized service plans, progress monitoring, and coordination with staff. #J-18808-Ljbffr St. Vincent De Paul
