Behavioral Health Consultant (Hybrid)

Job Details Location: Erie Division Street Health Center - 2418 W. Division St - Chicago, IL 60622 Position Type: Full Time Salary Range: $57,000.00 - $64,500.00 Compensation & Benefits Competitive salary of $57,000 to $64,500 per year, with annual merit increases. Full benefits package includes medical, dental, vision, life and disability insurance, FSA for health care or childcare, and a 401(k) program with a 50¢/1¢ match up to 5% of biweekly salary. Annual paid time off starts at 20 days plus 8 holidays. Main Duties & Responsibilities Clinical (approximately 80% of time): Brief Consultations: Perform an average of 8‑10 behavioral health consultations per day, ensuring at least 60% clinical time for warm hand-offs and same‑day visits. Individual, couples, or family consultations: Conduct functional and strength‑based assessments, psychoeducation, medication and disease self‑management counseling, motivational interviewing, and brief problem‑solving cognitive interventions. Self‑care plan development, skills training for disease self‑management, coping, distress tolerance, stress reduction, relaxation, and substance use/abuse evaluation with harm reduction strategies. Consultation with PCPs to support treatment planning and monitoring of biopsychosocial plans. Coordination of Care: Provide consultation to primary care staff, complete psychiatric intake evaluations, appropriate screenings, and refer patients to specialty behavioral health providers. Crisis Intervention: Available during working hours for psychiatric emergencies and other emergencies as part of the health center staff. Administrative (approximately 20% of time): Supervision: Participate in individual supervision, attend monthly clinical group supervision, and supervise 1st‑ and 2nd‑year interns as needed. Charting: Complete all EMR charting within 48hours of assessment, progress notes, and billing as required. Qualifications Education: Master’s degree in a behavioral health discipline (e.g., Social Work, Psychology, Mental Health Counseling) with a valid Illinois State licensure appropriate to the discipline. Required Skills and Knowledge Prior work experience as an LSW or LPC in a medical setting or Federally Qualified Health Center. Cultural competency to work in low‑income, minority environments. Ability to maintain confidentiality and trust with clients. Preferred Skills and Knowledge Knowledge and application of behavior change, adult learning, and group process theories. Bilingual fluency (English/Spanish) and bi‑cultural competency. Community resource orientation. Computer skills (MS Word, MS Excel, MS Publisher, etc.). Time‑management and multitasking skills.
