Description: The Community Health LPN provides and documents nursing care for the sick and injured in a home setting. The LPN intervenes to promote, maintain, or restore health, prevent illness, and effect rehabilitation. The LPN collaborates with other healthcare providers and community representatives to coordinate care at home. The LPN evaluates individual and family responses to interventions. Duties require independent action and judgment. The role involves time management, communication, planning, delegating, and problem-solving in providing care. Critical thinking skills are essential. The candidate must maintain emotional stability in emergency settings. Demonstrated ability to perform independently as a clinician with minimal supervision is required. Knowledge of home care reimbursement and regulatory guidelines is essential for effective service delivery. The candidate must have a history of dependability in meeting work schedule demands.
