RN Case Manager - Home Health

Job Description & Requirements StartDate: 8/24/2026 Pay Rate: $1800.00 - $2700.00 POSITION SUMMARY – RN Case Manager - Home Health Collaborate with the patient, their family, and the physician to develop and regularly update an individualized care plan, coordinating services through ongoing interdisciplinary communication. Assess the patient’s clinical, functional, and emotional status, documenting progress and reporting significant changes to the care team. Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions, while using each interaction as an opportunity for meaningful health counseling. Minimum Required Qualifications – Compact or VT & NH unencumbered RN license Graduate of an accredited school of nursing BLS 2 years of experience in Home Health settings
