Community Support Specialist
HEALTH CONNECT AMERICA, INC
Health Connect America is seeking a Community Support Specialist to provide behavioral health support to children, adults, and families in homes, offices, and community settings. In this role, you will implement treatment plans, teach coping and daily living skills, support crisis de‑escalation, and connect clients with community resources. You’ll collaborate closely with therapists, case managers, and community partners, document services accurately, and maintain confidentiality. We offer training, supervision, and growth opportunities in a mission‑driven culture focused on compassionate, client‑centered care and improved well‑being.
