Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Florida physical therapy license required prior to start of assignment.
• BLS certification from AHA or ARC may be required.
• Home health experience required, including performing evaluations, developing measurable goals, and documenting per payer and regulatory timelines.
• Ability to supervise Physical Therapist Assistants and aides in accordance with state practice act.
• Client-specific requirements: availability for visits primarily during daytime business hours (8:00–16:30) Monday–Friday with one weekend per month availability for weekend PT admits; productivity expectation of 25–30 visit ‘points‘.
• Experience with OASIS documentation and home care EMR systems preferred.
• Experience treating post-operative joint care preferred.

Impacting Patient Care

Perform physical therapy evaluations and treatments based on physician orders. Complete comprehensive evaluations using appropriate assessment tools. Establish measurable goals and develop treatment plans to maximize patient function. Reassess progress regularly and modify plans as needed. Communicate patient needs and progress with the care team and physicians. Recommend and assist with adaptive equipment. Maintain documentation in required formats and timelines. Adhere to HIPAA, company compliance policies, and applicable state and federal laws. Participate in staff development and maintain required continuing education.
