Care Specialist CCBHC

JOB DESCRIPTION Provides person-centered assistance to clients in the development of life skills and personal goals. Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their families. ESSENTIAL DUTIES & RESPONSIBILITIES Coordinate client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards. Teach and train clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provide services according to organizational guidelines and/or protocols. Connect clients with community support systems as determined to be appropriate, and maintain regular communications with schools, OFC’s and other family service providers that the client and their family engage with or is receiving services from. Provide telephone and on-site crisis intervention/prevention and follow-up to clients and their families. Understand how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model. Perform all required job duties and responsibilities in accordance with CCBHC Standards. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Customer service skills. Time management skills. Analytical skills. Knowledge of community resources. QUALIFICATIONS Associate's degree required. Two years of experience or two years of college coursework in related field may be substituted for degree requirement. Two years professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred. Certification/Licensure: N/A. TIME & PAY Time Type: Full time. Pay Range: $19.80–$23.00 per hour. BENEFITS Medical, dental, and vision health coverage.
