Licensed Practical Nurse | Long Term Care | Gallup, New Mexico, United States | Start: 09/01/2026 | Duration: 7 contract weeks | Weekly pay: $841-$873 per week | Guaranteed hours: 36 per week | Shift: 07:00–19:30 (12-hour day shifts) | Setting: Skilled Nursing Facility (long-term care) | This is a local, temporary assignment with a seven-week contract.

Responsibilities
- Administer prescribed medications and document according to policy and MAR
- Monitor residents, assess vital signs, and report changes to the supervising RN
- Provide direct patient care to support activities of daily living (ADLs), mobility, hygiene, and feeding
- Assist with wound care and dressing changes as ordered
- Collaborate with the interdisciplinary care team to implement and update care plans
- Maintain accurate documentation and participate in shift handoffs to ensure continuity of care

Requirements
- Active LPN license in the state of New Mexico in good standing (or eligible for NM licensure)
- Completion of an approved Practical Nursing program
- Prior experience in a long-term care or skilled nursing facility setting preferred
- Ability to work the listed shift hours (07:00–19:30) on a local assignment
- Strong communication and teamwork skills; proficient in basic documentation
- Must pass background check and drug screen as required and comply with facility policies
