Role Description The HCC Coder is responsible for accurately assigning Hierarchical Condition Category (HCC) codes based on clinical documentation to support risk adjustment and compliant reimbursement. In this full-time, on-site role located in Chanhassen, MN, the coder reviews medical records for completeness and clarity, interprets provider documentation, and applies appropriate ICD codes according to current guidelines. Day-to-day tasks include validating diagnoses, identifying documentation gaps, and collaborating with clinical and coding leadership to resolve queries and improve data integrity. The HCC Coder also participates in ongoing quality audits, follows organizational and payer-specific coding policies, and supports continuous improvement initiatives in coding accuracy and productivity. Qualifications Candidates should possess strong Medical Coding and Coding Experience, including familiarity with ICD coding and risk adjustment methodologies. Candidates should possess knowledge of Health Information Management principles and practices, particularly as they relate to coding and documentation standards. Candidates should possess a solid foundation in Medical Terminology to accurately interpret clinical documentation and apply appropriate codes. Candidates should possess relevant credentials such as RHIT or other recognized coding certifications (e.g., CPC, CCS, CRC) that demonstrate professional competence.
