Responsibilities
- Administer prescribed medications and monitor effects, per physician orders and facility policy
- Perform routine and complex nursing care for residents, including activities of daily living
- Monitor patient status, vital signs, and respond to changes in condition; escalate to team members as needed
- Document care provided in the electronic medical record and maintain accurate, secure records
- Support wound care, dressing changes, IV administration as trained and permitted
- Collaborate with the interprofessional care team to ensure quality, timely care and resident safety

Requirements
- Active NYS Licensed Practical Nurse license
- Basic Life Support (BLS) certification
- Minimum 2 years of practical nursing experience in a SNF/long-term care environment
- Education: LPN diploma or equivalent
- Ability to work the stated 5x8 evening shift and to meet the requirement of every other weekend; must be able to work at least 4/8 of a shift if needed
- Acceptable for local travel within 50 miles; local travelers may have a decreased bill rate
- Willingness to work in a long-term care setting and adhere to facility policies
