About the Role:

• As an LPN, you will work under the supervision of our registered nurses and physicians to provide direct patient care, including administering medications, monitoring vital signs, and assisting with daily living activities.

• You will also be responsible for maintaining accurate and detailed records of patient care, communicating with healthcare teams, and participating in patient education and discharge planning.

Responsibilities:

• Provide high-quality, patient-centered care to our residents and patients, ensuring their physical, emotional, and social needs are met.

• Administer medications and treatments as prescribed by physicians and as directed by registered nurses.

• Take and record vital signs, including temperature, blood pressure, pulse, and respiratory rate.

• Assist with daily living activities, such as bathing, dressing, and grooming.

• Maintain accurate and detailed records of patient care, including medications, treatments, and test results.

• Communicate effectively with healthcare teams, families, and patients to ensure seamless care transitions.

• Participate in patient education and discharge planning, promoting independence and self-management.
