Supervisor, Transitions of Care Coordinator

This role provides frontline leadership for a virtual team supporting patients as they transition from hospitals or post-acute settings back to home. You will guide care coordinators who help patients access essential services, appointments, transportation, supplies, and durable medical equipment. The position combines people leadership with hands-on patient support, with approximately 30% of time dedicated to direct transitions-of-care activities. You will help strengthen workflows, develop job aids, monitor performance, and coach team members toward high-quality outcomes. The role also offers the opportunity to contribute to program development, cross-functional initiatives, and evolving care models. Success requires an organized, empathetic, technology-savvy leader who can balance operational performance with a strong patient-centered approach.

Accountabilities:

• Lead and support a virtual team of Transitions of Care Coordinators, providing day-to-day guidance, answering questions, conducting 1:1s, monitoring communication channels, and fostering a collaborative and patient-focused environment.
• Dedicate approximately 30% of working time to direct transitions-of-care patient support, with responsibilities adapting based on team size and operational needs.
• Oversee team performance across key performance indicators, attendance, adherence, meeting participation, and other operational expectations, while identifying opportunities for improvement.
• Support patients during transitions from hospital or post-acute settings to home by helping coordinate appointments, transportation, supplies, durable medical equipment, and follow-up communication.
• Partner with leadership to develop the transitions-of-care program, represent coordination functions in cross-functional meetings, and communicate relevant updates and recommendations to broader teams.
• Assist with recruiting and hiring new coordinators and partner with training and education teams to support onboarding and professional development.
• Contribute to process improvement initiatives by designing and implementing new workflows, creating job aids, facilitating training, supporting care-model pilots, and making recommendations to senior leadership based on team performance.

Requirements:

• Medical Assistant (MA) certification is desired, and expired certifications may be accepted; candidates without certification but with significant MA experience may be asked to provide relevant education documentation such as a school diploma and transcripts.
• At least 2 years of direct leadership or supervisory experience in a virtual work environment is required, with demonstrated ability to coach, manage performance, and support distributed teams.
• At least 2 years of healthcare administrative or office experience is preferred, along with familiarity with patient support and healthcare coordination processes.
• Strong technology skills and the ability to adapt quickly to evolving digital platforms, communication tools, and operational systems.
• Highly organized with exceptional attention to detail, follow-through, and the ability to manage multiple priorities in a fast-paced remote environment.
• Strong communication, collaboration, and problem-solving abilities, with the interpersonal skills needed to work effectively with patients, team members, clinical partners, and organizational leadership.
• Fully bilingual English/Spanish communication skills are preferred but not required, particularly for supporting diverse patient populations and care teams.
