Transitional Care Coordinator

The Transitional Care Coordinator acts as a vital program liaison, overseeing patients throughout the entirety of their service offerings, from admission to post-discharge care and outpatient referrals. Through both telephonic communication and in-person visits, the transitional care coordinator is dedicated to ensuring that hospitalized patients undergo a seamless transition from an acute care facility to their home environment.

The Transitional Care Coordinator serves as the patient's liaison throughout the life cycle of the program by addressing program specific quality measures and adhering to company guidelines/standard operating procedures. The Transitional Care Coordinator also travels to hospitals to conduct in-person visits with patients in the acute or subacute setting and/or complete telephonic visit with patients. Confirms member demographics, review patient equipment history, intakes acute facility order, and overall supports patients based on identified needs. Accurately documents all phone calls, interventions, appointments, visits and interactions with client, healthcare provider, patients in the company system within 24 hours. Reviews company system related tasks throughout the day for management of daily responsibilities to effectively and thoroughly manage all assigned patient cases to completion. Proactively assist with locating community resources via discovery, research, documentation, networking, training, etc. of community/social resources solutions for patients. Maintains hospital credentialing through the hospital credentialing website and regularly check to ensure that credentialing documentation is valid and compliant. Adheres to hospital standards of conduct, policies, and procedures. Performs other duties and responsibilities as required, assigned, or requested.

Experience/Skills:

• Associate degree or higher from an accredited college, preferred.
• 2 years of strong customer service and healthcare experience in a fast-paced environment.
• Experience with Home Health, and Durable Medical Equipment (DME).
• Knowledge of medical terminology and healthcare landscape, preferred.
• Proficient with Microsoft Office applications including Word, Excel and PowerPoint.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year
