Job Summary:
The Speech Therapist provides treatment to the patient to improve sensory-motor development, perception, memory. language and cognition.

Responsibilities include:
Provides initial and ongoing interdisciplinary assessment of the patient to identify the patients level of functioning, swallowing and communication ability.
Provides speech therapy in compliance with practitioner orders, patients plan of care, FLHC policies and procedures and federal and state guidelines
Develops a plan of care in consultation with the health care practitioner and other team care members.
Instructs patients and families on proper consistency of food to maintain nutritional intake and prevent complications of swallowing disorders.
Instructs patients and families on using special educational strategies and techniques to improve sensory-motor development, perception, memory, language and cognition.
Develops appropriate home exercise programs tailored to the patients clinical status, needs and age.
Documents all care and services provided according to professional standards and agency policy and procedures.
Adheres to all agency policies and procedures.
Completes all mandatory agency training and in-service activities.
Participates in the agencys quality management/QAPI activities.

The ideal candidate will have:
Knowledge of speech therapy practice, with emphasis on the provision of physical therapy services in the home environment.
Ability to foster constructive and therapeutic relationships with patients/clients and their families.
Ability to communicate effectively, both orally and in writing.

EDUCATION/EXPERIENCE
Licensed as a Speech Pathologist
Graduate of an approved/accredited college program that meets the educational requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association.
Two years experience as a Speech Therapist. Previous experience in the home care or outpatient therapy industry preferred.

CERTIFICATIONS, LICENSURE, & REGISTRATION
Current, valid license to practice as a Speech Pathologist.
CPR certification.
Current, valid driver's license.
