Catholic Community Services, Family Behavioral Health is a values‑driven organization offering community‑based case management for children, youth, and families. This position is a Mental Health Care Coordinator (Internal title: Clinical Care Coordinator) based at our Salmon Creek office in Vancouver, WA. Position Summary Provide individualized, evidence‑based, community‑based case management and therapeutic mental health services in families’ homes and community settings. Participate in an on‑call rotation with other clinicians. Responsibilities Meet and engage families with compassion, support, and enthusiasm. Arrange and provide crisis response and stabilization services, creating and communicating safety plans. Prepare families for the next steps in intervention, including plain‑language plans and action steps. Customize therapeutic approaches to fit each family’s unique culture, values, and strengths. Provide intensive, strength‑based therapeutic services to children, youth, and families. Assess safety and stabilization needs, develop treatment plans, and facilitate family team meetings. Perform ongoing assessment and evaluation of treatment planning strategies, goals, and outcomes. Brainstorm and negotiate strategies that build on family strengths. Consult with supervisors and specialists, incorporating recommendations. Meet documentation standards and maintain accurate, strength‑based records. Respond promptly to crises in a collaborative manner. Facilitate individual and family therapies and utilize parent/family expertise in problem solving. Adjust intervention techniques as needed while maintaining the plan’s direction. Lead team implementation of intervention plans from initiation to redesign. Arrange formal and informal services and supports, and plan for after‑care. Participate in on‑call rotation with clinical care coordinators. Compensation Starting pay: $33.06 – $36.78 per hour. First Responder bonus: $1,800 per year. Performance‑based pay: up to $8,000 per year. Training & Supervision Extensive training in multiple clinical approaches and related areas. Weekly supervision and daily support from your Clinical Supervisor. Benefits 12 paid holidays plus 1 personal holiday per year. 3 weeks of vacation per year. 12 sick days per year. Flexible schedule preference over traditional 8‑5 weekday position. Availability for on‑call rotation responsibilities. Successful completion of required background clearance.
