Position Social Services Coordinator Shift Schedule 8:30 AM to 4:30 PM Duties Conduct resident interviews to gather information about social, emotional, and financial needs and concerns. Assist in discharge planning by coordinating with healthcare providers, families, and community resources to ensure a smooth transition for residents. Document interviews, phone calls, and care conferences on a daily basis in accordance with facility policies and procedures. Maintain regular communication with interdisciplinary team members, including nurses, therapists, and social workers, to ensure effective care planning and coordination. Adapt daily schedule to accommodate the needs of staff, residents, and families. Provide Social Services oversight completing duties as assigned for the residents of Eden Senior Care. Work closely with Marketing Director to coordinate referral‑to‑admission process. Communicate with insurance companies to obtain prior authorization for admissions; keep record of insurance updates and communicate with departments to ensure timely completion. Assist new residents and family members with completion of admission paperwork. Provide information to residents/families regarding Medicare, Medicaid and other financial assistance programs; assist with Medicaid applications as needed. Ensure PASRR is completed to state requirements. Participate in resident assessment – provide background information and social history as identified in the MDS process, in accordance with the RAI manual. Involve the resident and family member in planning objectives and goals; develop a written plan of care to work toward meeting identified goals. Attend and lead behavior meetings and monitor gradual dose reductions (GDR) and consents for psychotropic medications according to state regulations. Complete timely progress notes on each resident. Maintain an open‑door policy for residents, family members, and staff to discuss problems or needs; provide counseling or referrals to meet those needs. Report concerns or grievances to the Executive Director and initiate investigations. Serve as an advocate for the resident and their rights, helping them learn assertiveness in communicating needs, wants, and problems to the appropriate departments and agencies. Participate in monthly QAPI Committee meetings. Attend daily stand‑up meetings; report to IDT about pay changes, care conferences, concerns, etc. Initiate and lead the discharge planning process; coordinate with outside resources such as DME, home health, hospice, etc. to ensure a safe transition to home or lower‑care setting. Issue denial letters according to state regulation. Assist in providing in‑service education and orientation for employees regarding resident rights and social service needs. Perform all other duties assigned by Administrator. Requirements Preferred: Bachelor’s degree in Social Work or related Human Services field – Social Services, Sociology, Special Education, Rehabilitation Counseling, Psychology. Licensed Social Work certificate in Pennsylvania (current and valid is a huge plus). One year of supervised social work experience in a healthcare setting working directly with individuals (preferred but not required). Ability to promote cooperation between social services and other department employees. Professional conduct over the phone and in person when representing the company. Excellent communication skills and pride in performance. Ability to write and give clear, concise instructions and remain calm during emergency situations.
