Job Title: LICENSED Nurse (LPN) – Adult Day Care Medical Activity Center (FULL TIME)

Location: Holmdel NJ,

Schedule: FULL TIME

Pay: Negotiable based on experience

Position Overview:

We are newly opening Adult Medical Senior Center / Daycare seeking a compassionate and experienced LPN to join our Adult Day Care Medical Activity Center. This role focuses on supporting the health, safety, and well-being of senior participants in a structured, community-based setting. The ideal candidate has strong clinical skills, works well with elderly populations, and understands the Medicaid process.

Key Responsibilities:

Conduct health assessments and monitor participants’ medical conditions

Administer medications and treatments as prescribed

Maintain accurate and up-to-date medical records

Coordinate care with physicians, families, and interdisciplinary staff

Respond to medical needs and emergencies in a timely manner

Assist with care planning and ensure compliance with state and local regulations

Support daily activities while promoting a safe and engaging environment

Provide guidance and education to staff and caregivers.

Qualifications:

Active Registered LPN license in the state of NJ

Minimum 2 years of nursing experience, preferably with seniors or in a similar setting

Knowledge of Medicaid processes and documentation requirements

Strong interpersonal and communication skills

Ability to work independently and as part of a team

Compassionate, patient, and attentive to the needs of elderly individuals.

This job does not come with benefits, paid holiday or paid vacations.

It is hourly plus bonuses.

Preferred Experience:

Adult day care, geriatrics, long-term care, or community health experience

Familiarity with state regulations for adult medical day programs.
