Job Summary:
We are seeking a compassionate Medical Social Worker to support home health patients and their families by addressing psychosocial, emotional, and resource needs. We hire the most compassionate and thoughtful talent in the field. Come join an amazing agency that values their employees.
Responsibilities:
Assess patient psychosocial and environmental needs
Assist with discharge planning and long-term care arrangements
Provide counseling and emotional support to patients and families
Connect patients with community resources and financial assistance programs
Complete required documentation per Medicare/home health standards
Participate in care coordination with interdisciplinary team
Qualifications:
Master’s degree in Social Work (MSW)
Active Florida license (LMSW/LCSW preferred)
Home health or healthcare experience preferred
Strong knowledge of community resources
Excellent communication and organizational skills
