Job Description
Speech-Language Pathologist - Home Health

Schedule

Self-scheduled visits, with a requirement of 6-8 home skilled visits per day. Availability for 1-2 weekends per contract.

Assignment Details

Work requires travel across multiple territories. Contract lengths vary based on the needs of the office.

Requirements

• 1 year of home health experience required
• Valid driver's license and personal vehicle for home visits
• Certification in BLS and CCC-SLP
• State Licensure required
• OASIS training preferred
• COVID vaccination or exemption form on file
• Flu shot or declination documented, with mandatory mask wearing while at work

Responsibilities

• Conduct evaluations using various screening tools such as BIMS, depression screening, and oral peripheral exams
• Develop and implement dysphasia management and aural rehabilitation plans
• Educate patients and families regarding therapy plans and patient safety
• Maintain documentation and billing in compliance with regulations
• Provide in-service training as required
• Implement functional maintenance programs for patients
