*What You'll Do*

* Provide mobile primary care to geriatric patients in assisted living, memory care, and adult family homes
* Diagnose, treat, and manage acute and chronic medical conditions
* Manage chronic diseases including CHF, COPD, diabetes, hypertension, dementia, and more
* Collaborate with caregivers, families, specialists, hospice, and home health partners
* Utilize our robust EMR and clinical support team to deliver exceptional patient care

*What We're Looking For*

Active Washington Nurse Practitioner License

DEA License (or ability to obtain within 90 days)

Current BLS/CPR Certification

Passion for caring for older adults

Excellent communication and relationship-building skills

Reliable transportation and a home office for occasional telemedicine visits
