Position Summary

As a Qualified Professional (QP) with Pinnacle Place, LLC, you will play a vital role in creating pathways to recovery and self-sufficiency for the individuals and families we serve. This position requires a skilled professional with at least 3 years of experience who is ready to contribute to our mission of improving quality of life through tailored, supportive services.

Key Responsibilities

In this role, you will:
- Provide person-centered support to individuals and families, addressing their unique needs and goals.
- Develop and implement individualized service plans using evidence-based practices.
- Collaborate with community partners to connect clients with resources that promote stability and independence.
- Monitor progress and adjust care plans as needed to ensure positive outcomes.
- Uphold the values of dignity, respect, and accountability in all interactions.

Required Skills and Qualifications

To succeed in this role, you’ll need:
- A minimum of 3 years of experience in a related field.
- Strong knowledge of behavioral health, social determinants of health, and supportive services.
- Excellent communication and interpersonal skills.
- A commitment to person-centered care and evidence-based practices.
- The ability to work collaboratively with a diverse team and community partners.
