Clinical Responsibilities

Conduct admissions and new patient evaluations

Manage chronic and acute medical conditions in the LTC setting

Perform routine follow-ups and regulatory visits

Collaborate with NPs, nursing staff, therapists, and facility leadership

Communicate with families and caregivers

Document encounters using a company-issued mobile device

The focus is on quality, coordination, and patient-centered care within a structured post-acute environment.

Daytime hours

Minimal on-call rotation

No regular nights or weekend rounding
