Our physician led medical group enjoys an excellent reputation with patients and the medical community, offering primary care, urgent care and more than 30 different specialties. \n Job Description \n Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager. \n \n Provides office-based nursing care in collaboration with provider \n Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention \n Communicates with provider regarding patient needs, nursing assessments, and recommendations \n Takes direction and initiates actions (cross/additional training) that will allow seamless patient care \n Collects and documents current health status and medical history and assists in scheduling patient office visits \n Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider \n Demonstrates independent nursing actions based on assessment and problem identification \n Under direction of provider, calls in prescription medications to pharmacies and observes and documents patient’s response to pertinent medications \n Documents accurate and ongoing assessment of patient’s needs, problems, capabilities, limitations, progress toward goals, and response to care \n Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner \n Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy \n \n Qualifications\n \n \n \n Current Connecticut RN License required \n \n Travel required to statellite locations as needed for coverage \n \n We take great care of careers. \n With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
