Role Description This is a full-time (but part-time can also be discussed), on-site role for a Nurse Practitioner based in San Jose, CA. The Nurse Practitioner will provide advanced medical care to patients, including diagnosing and treating illnesses, managing patient care plans, and offering preventative care. Responsibilities include conducting physical examinations, analyzing test results, prescribing medications, and coordinating with other healthcare providers for comprehensive patient care. The role requires collaboration with the healthcare team to ensure high standards of patient care and effective treatment plans. We serve diverse ethnic populations that include Korean and Hispanic groups. Qualifications Clinical expertise in diagnosing and treating patients, conducting physical assessments, and managing medical conditions, particularly in the context of Primary Care & Preventive Health. Familiarity with prescribing medications and designing individualized care or treatment plans. Strong interpersonal and communication skills for patient education and collaboration with a multidisciplinary healthcare team. Proficiency in managing electronic health records (EHR) and utilizing technology in medical care. Completion of a Nurse Practitioner program and licensure as a Nurse Practitioner in the state of California. Advanced training or experience in cardiovascular health is highly preferred.
