Perform hands‑on physical therapy sessions and document progress. Adhere to safety, compliance, and professional ethics in all activities. Set measurable patient outcomes and track progress against targets. Collaborate with multidisciplinary team to optimize rehabilitation plans. Utilize evidence-based protocols to ensure high-quality care and consistency. Maintain accurate, timely documentation in electronic health records. Educate patients on exercises, injury prevention, and home programs. Identify opportunities for professional development and pursue certifications. Engage in pipeline and territory planning to optimize patient referrals. Demonstrate accountability for individual outcomes within a team environment. Experience/Qualifications Licensed Physical Therapist with active state license and clinical competencies. Evidence-based assessment, treatment planning, and outcome measurement skills. Hands‑on therapy delivery and accurate progress documentation in EHR. Safety, compliance, and professional ethics adherence in all sessions. Ability to set measurable patient outcomes and monitor progress against targets. Strong collaboration with multidisciplinary teams to optimize rehab plans. Maintain high-quality care via evidence-based protocols and standardized procedures. Patient and caregiver education on exercises, injury prevention, and home programs.
