Carease Health - PA - Nurse Practitioner Job Summary: We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Carease Health. As a Nurse Practitioner with our organization, you will have the opportunity to deliver high-quality preventative care to residents in skilled nursing facilities, while enjoying a hybrid work arrangement that combines on-site visits with remote work from home. Our team is dedicated to improving the lives of nursing home patients through remote monitoring, chronic care management, and psychiatric collaborative care.
