About This RoleThe RN Clinical Care Coordinator will serve as the primary care manager for members with complex medical and behavioral needs, focusing on care coordination to support their medical, behavioral, and socioeconomic requirements. Highlights Pay: $40.00 - $54.00 per hour Location: East Brunswick, NJ Department/Unit: Field-Based Schedule: Full Time Travel: Ability to travel up to 80% of your time within assigned territory What You’ll Do Engage members face‑to‑face and/or telephonically to complete a comprehensive needs assessment. Develop and implement person‑centered care plans addressing chronic health conditions and wellness. Partner and collaborate with internal care teams, providers, and community resources. Provide education and coaching to support member self‑management of care needs. Support proactive discharge planning and manage care transitions following ER visits or admissions. Advocate for members and families to ensure their needs and choices are represented. Requirements Education: Current, unrestricted independent licensure as a Registered Nurse in New Jersey Experience: 2+ years of clinical experience 1+ years of experience with MS Office, including Word, Excel, and Outlook Licenses & Certifications: Current RN License in New Jersey required Driver’s license and access to reliable transportation #J-18808-Ljbffr IntelliResume
