Oak Street Health seeks a Primary Care Physician to provide comprehensive, patient-centered care for adults on Medicare in underserved communities. In this role, you’ll manage a dedicated patient panel, focus on prevention and chronic disease management, and collaborate with an interdisciplinary care team to improve outcomes. You’ll leverage data and innovative care models to reduce hospitalizations and enhance quality of life.
