Health System Hospice Liaison

VITAS Healthcare is seeking a field-facing professional to build and maintain relationships with health systems and patients/families aligned with our mission. You will educate medical staff, patients and families about hospice services and deliver end-of-life care solutions to improve timely access to care. You will become a subject matter expert on Medicare Hospice Benefit, VITAS offerings, and clinical criteria, coordinating care across teams and ensuring smooth admission processes and #J-18808-Ljbffr VITAS Healthcare
