In your role, you will evaluate patients in inpatient or outpatient settings, craft individualized treatment plans tailored to their unique needs, and administer crucial physical therapy interventions that enhance mobility, strength, and overall function. Working collaboratively with a dedicated team that includes nurses, physicians, and case managers, you will strive to optimize patient outcomes through coordinated care. Your responsibilities will extend beyond treatment; you'll educate patients and their families about therapeutic goals, discharge plans, and home exercises. With an unwavering commitment to documentation, you will meticulously track evaluations and progress in accordance with established guidelines while participating in interdisciplinary conferences to ensure the highest quality of care.

You'll be equipped with a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution, along with state licensure as a Physical Therapist, and possibly, CPR certification. Specialized training in areas such as neurology or geriatrics can be advantageous, but it's not mandatory. Ideal candidates will have previous experience, possess exceptional ability to manage patients with diverse medical backgrounds and mobility challenges, and exhibit outstanding communication skills. Comfort with digital documentation systems is also expected.
