Hospice Registered Nurse Case Manager

Hebrew SeniorLife, a Boston-based nonprofit affiliated with Harvard Medical School, is seeking a Hospice Registered Nurse Case Manager to provide compassionate, expert care for older adults. In this 40-hour role, you will manage a caseload of hospice patients, lead individualized care planning, and coordinate services across an interdisciplinary team. You’ll conduct ongoing assessments, manage complex symptoms, and support families through end-of-life decisions.
