The PT will deliver care in accordance with a physician's plan, helping patients regain mobility and independence.ResponsibilitiesEvaluate patients' functional status and develop care plansAssess home environments and recommend modificationsProvide direct physical therapy treatmentsEducate patients and caregivers on therapeutic techniques and equipmentMonitor progress and adjust treatment plansCollaborate with physicians and interdisciplinary teamsDocument clinical notes and progress reportsParticipate in discharge planningSupervise Physical Therapy Assistants per policy and regulationsQualificationsActive California Physical Therapy LicenseMinimum 2 years of physical therapy experience (home health preferred)Strong communication and organizational skillsCurrent CPR certificationValid driver's license and insured, reliable vehiclePay Range: $50 - $65 per hour
