Full-Time Pediatric Registered Nurse/RN Care Coordinator

New Alternatives for Children, Inc. (NAC) is an award-winning health care and social service agency in Midtown Manhattan which serves children living with medical complexity, disabilities or serious emotional or psychiatric disturbance and their families. We are currently seeking a Pediatric Registered Nurse to work in the communities of families enrolled in our Prevention and Foster Care programs and in our clinics. It's an exciting opportunity to be a part of multidisciplinary teams of professionals at an innovative agency serving the needs of some of the most vulnerable children and families in New York City.As a member of NAC's nursing team, the Pediatric Registered Nurse (hereafter referred to as, "nurse") is responsible for a caseload of children and youth (hereafter referred to as, "children") and families. The nurse makes clinical assessments during visits in the community and the clinics; provides anticipatory guidance and health-related education to children and families; and helps coordinate care for children living with medical complexity, disabilities or serious emotional or psychiatric disturbance and their families. The nurse provides these services during regularly scheduled visits to families' homes and other community sites, and on-site in our Article 28 and 31 clinics. Care is delivered in collaboration with NAC's outstanding teams of social workers, behavior specialists, mental health professionals, education specialists, physicians, nurse practitioners, and other staff.In addition, the nurse helps staff NAC's Article 28 Clinic on a scheduled basis and, when possible, during scheduled appointments with children on the nurse's caseload.Position Title: Pediatric Registered NursePosition Type: Full Time (35 hours per week, with a flexible schedule)Salary Range: $80,000-$86,000Location: 825 7th Ave New York NY 10019Responsibilities in the Community:Make home visits to children in the Prevention and Foster Care Programs, according to program guidelines. In response to the COVID-19 pandemic, and as a way to reduce transmission of SARS CoV-2 in the community, some contacts with children and families will be made virtually through the use of telephone and video communication in lieu of in person visits to the homeAccompany children and families to selected primary care and pediatric and surgical subspecialty visits, as neededAttend selected multidisciplinary conferences held in community settings around the health care needs of children and families. In response to the COVID-19 pandemic, and as a way to reduce transmission of SARS CoV-2 in the community, these conferences will primarily be held virtuallyResponsibilities in the Clinics:Help oversee clinic activities to ensure that encounters occur in a timely fashionObtain and document vital signs, perform venipuncture and collect specimens for various labs on children and adults served in the clinicsPerform vision and hearing screening and administer immunizations for children served in the clinics, as ordered by MD/PNPAssess and triage children and families presenting with concerns for acute illnessProvide direct care to patients that includes (but is not limited to) providing treatment for asthma, conducting physical assessment in cases of suspected child maltreatment, and delivering emergency careAssist with the cleaning and disinfection of exam rooms in accordance with policies/procedures and guidelines set forth by the CDC, NYS Department of Health and NYC Department of Health and Mental HygieneOther Care Coordination Activities:Assess the medical needs of each child by reviewing diagnoses, birth history, and inpatient and outpatient recordsProvide education for families on how to optimally care for their children in their home settings, particularly around the children's special health care needs, with reinforcement as neededCollaborate with social workers, education specialists, recreation staff, mental health and other medical professionals to assess each child's health care needs and problems and implement health care plans. Maintain and update those plans as needed, assuring optimal quality of careParticipate actively in multidisciplinary Clinical Care Conferences and Family Team Conferences convened at NAC and selected interagency case conferences held at NAC and in the communityMake referrals for community-based care and provide follow-up as neededProvide follow-up to all children taking psychotropic medications. Serve as liaison to mental health professionals at NACOther Duties:Ensure accurate and timely documentation of all patient care and community based activitiesProvide telephone triage and advice to parentsAssist in administrative duties, when neededAssist in quality improvement activities involving services provided to children in assigned caseload, including maintaining up-to-date medical record documentationRequired Education and ExperienceCurrent New York RN license (in good standing) requiredMust be a graduate of an accredited school of nursing.
