Temporary Clinical Review Pharmacist

Immediate Need!


We are seeking a qualified Pharmacist to assist the Veterans Home in Ardmore, OK on a temporary assignment.

The position will be responsible for ensuring the safe, effective, and appropriate use of medications for veteran residents. This role involves reviewing patient medication regimens, monitoring for drug interactions and adverse effects, providing medication therapy management, and collaborating with healthcare providers to optimize treatment plans.


Key Responsibilities:

• Conduct comprehensive medication reviews for veteran residents.
• Identify and resolve medication-related issues, including interactions and contraindications.
• Provide drug information and consult with physicians, nurses, and other healthcare staff.
• Ensure compliance with VA policies, federal regulations, and best pharmacy practices.
• Participate in interdisciplinary team meetings to support patient care planning.
• Educate staff and patients on proper medication use and safety.

Qualifications:

• PharmD or BSPharm degree from an accredited institution.
• Active pharmacist license (state-specific requirements may apply).
• Experience in geriatrics, long-term care, or clinical pharmacy preferred.
• Knowledge of VA healthcare systems and regulations is a plus.
