Field-Based RN Case Manager: Wound Care & Infusions

Adaptive Home Health

Adaptive Home Health is seeking a Registered Nurse Case Manager to deliver high-acuity home health care in the field across Texas. You will manage wound care and infusion therapies, develop plans of care, and advocate for patients and families while coordinating with physicians and the care team.

The role emphasizes autonomous practice, a clear productivity model with upside, and schedules designed for field work sustainability. A frontline position improving patient access to home health care.
