Join us in our mission to enhance the well-being of our patients and residents while making a meaningful impact in the communities we serve.ResponsibilitiesEnrich lives as a nursing center social services specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
