NURSE (BEHAVIORAL HEALTH CARE FACILITATOR)

Overview Position: Nurse (Behavioral Health Care Facilitator) – Full Time, Fort Sill, OK. Located at Reynolds Army Health Clinic. Responsibilities Care Facilitation and Communication This role involves independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in treating patients with mental health conditions. The facilitator promotes continuity of care across a multidisciplinary team for depression, PTSD, and anxiety, monitors progress using validated tools (PHQ‑9, GAD‑7, PCL‑5), provides education, helps develop SMART self‑management goals, ensures treatment plan adherence, documents in the electronic medical record, reports abnormal results, and coordinates referrals to internal and external community services. Emergency situations are also addressed. Education and Caseload Management Assists the clinic with patient and staff education and delivers individual and small‑group training to increase referrals and understanding of the facilitator’s role. Engages in activities to grow caseloads, such as reviewing PCM appointments and population metrics, supports the Mental Health Consultant with educational classes, compiles and maintains diverse data reports, and participates in meetings and performance improvement activities in collaboration with the Nurse Case Manager. Quality Assurance Performs quality improvement and internal control activities, including risk appraisals, chart reviews, trend identification, and corrective action reporting. Participates in quarterly peer reviews of chart documentation with other BHCFs. Perform other duties as assigned. Qualifications Who May Apply U.S. Citizens Basic Requirements for Nurse (Behavioral Health Care Facilitator) Degree: A graduate or higher level degree, bachelor’s degree, associate degree, or diploma from an accredited professional nursing educational program is required. (Applicants must attach a copy of transcripts.) License: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a U.S. territory; and must have passed the National Council Licensure Examination. Foreign Degree: A degree from a foreign nursing school; official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Exemption: Exempt if a current federal employee in the 0610 occupational series employed continuously since September 27 2017 or earlier. Exempt applicants must provide documentation. Exempt conditions include: Degree or diploma from a professional nursing program approved by the state accrediting agency at program completion. Active, current registration as a professional nurse in a State, District of Columbia, Commonwealth of Puerto Rico, or a U.S. territory. Additional Qualification Requirements Education/Experience: A diploma, associate degree, or bachelor’s degree in nursing from a professional nursing program and at least one full year of professional nursing experience in an ambulatory or inpatient setting, including supporting patient adherence, providing education, and assessing symptoms against standard measures. OR Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. (Applicants must attach transcripts.) Exemption: Exempt if a current federal employee in the 0610 occupational series employed continuously since September 27 2017 or earlier. Exempt applicants must meet one of the following: Completion of all requirements for a doctoral degree (PhD or equivalent). Three full years of progressively higher‑level graduate education from a professional nursing program. Foreign Education: If foreign education is used, credentials must be evaluated by an organization that specializes in foreign education interpretation and deemed equivalent to a U.S. accredited program, or full credit granted at a U.S. accredited institution.
