Home Care Service Coordinator (Bilingual - Scheduler)

Full Time Service Coordinator – Albuquerque, NM Ambercare is seeking a full‑time Service Coordinator to schedule and supervise in‑home care workers and clients in a geographic area. Responsibilities include conducting in‑home visits, monitoring and reporting quality of services, and ensuring care plans meet client needs. Schedule: Monday through Friday, 8:00 AM – 5:00 PM. Responsibilities Schedule home care workers in the system; manage changes due to client requests, illness, vacation or leaves of absence. Provide alternate coverage to maintain care plan compliance. Contact care providers and clients to provide service updates. Monitor clients’ condition and Home Care Aides’ performance. Maintain all company and Department on Aging rules, regulations, and standards. Conduct home visits to welcome new clients and review the Welcome Packet. Observe and evaluate Home Care Aides in the client’s home as directed by Supervisor to ensure Care Plans are followed. Prepare and complete accurate evaluation reports and case notes. Maintain schedules to ensure timely completion of all assigned home visits. Attend weekly branch meetings. Maintain a high degree of confidentiality regarding sensitive information. Maintain regular, predictable, consistent attendance and flexibility to meet the department’s needs. Follow Medicare, Medicaid, and HIPAA regulations and requirements. Abide by all regulations, policies, procedures and standards. Perform other duties as assigned. Qualifications Minimum of 2 years’ experience working in senior services or related social service field. Excellent oral and written communication skills. Strong interpersonal skills. Ability to prioritize and handle high call volumes. Client-focused and customer‑service driven. Proficient with Microsoft Word, Excel, and Outlook.
