Your Contributions Ensure all activities align with Elara Caring’s vision and the Home Health team’s leadership. Assume primary responsibility for caseload: assess, plan, coordinate, implement, and evaluate care. Maintain patient and staff privacy in accordance with HIPAA. Assess case load, identify needs, incorporate data into care plans. Perform initial evaluation and re‑evaluate during visits. Communicate findings to supervisors, physicians, facilities, and team members. Revise care plans as needed and notify relevant personnel. Deliver care while adhering to safety and professionalism. Qualifications Current, unrestricted state RN license. 1+ year clinical care experience. Ability to lift, push, pull, and support up to 50 lbs, including patient transfers. Willingness and ability to travel up to 50% within the coverage area. Dependable vehicle, valid driver’s license, and current auto insurance. Associate’s or Bachelor’s degree in Nursing preferred. Previous home health or hospice experience preferred. Experience with HomeCare HomeBase preferred. Compensation and Benefits The base salary for this position is $70‑$75 per point, based on company estimates. Pay will be determined by education, experience, skills, and internal equity.
