We are seeking an experienced Discharge Planning Coordinator (LPN) to oversee the discharge process for our Sub-Acute Unit and help residents transition safely and successfully to the next level of care.
Responsibilities of a Discharge Planning Coordinator (LPN) :
• Coordinate and oversee resident discharge planning for the Sub-Acute Unit
• Participate in utilization review and insurance/Medicare reviews
• Issue Medicare cut letters as needed
• Collaborate with interdisciplinary teams on treatment and discharge plans
• Provide social service support to residents and families
• Coordinate community resources and post-discharge services
• Maintain accurate records and documentation
Qualifications:
• Master’s Degree in Social Work
OR
• Active NJ LPN or RN license
• Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning
• Long-term care or rehab experience preferred
