Position Summary Candidates must be located in Miami Dade county, specifically zip code 33016. This role will require 50‑75% travel for face‑to‑face visits with members in Miami Dade. Schedule is Monday‑Friday, 8:00am‑5:00pm, standard business hours. We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our Case Management team. Case Management Coordinator responsibilities include: Telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case‑management activities with members to evaluate their medical needs and facilitate overall wellness. Managing a caseload that includes supportive and medically complex members. Developing proactive courses of action to address issues presented and enhance short‑term and long‑term outcomes. Determining appropriate services and supports based on members’ health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, community resources, and supports. Required Qualifications Bilingual in English and Spanish. Willing and able to travel 50‑75% of their time to meet members face‑to‑face in Miami Dade county and surrounding areas in FL. Must reside in zip code 33016. 1+ year of experience in behavioral health or a long‑term care setting. Computer proficiency in Microsoft Word, Excel, Outlook. Preferred Qualifications Ability to multitask, prioritize and effectively adapt to a fast‑paced changing environment. Effective communication skills, both verbal and written. Managed Care experience. Case management and discharge planning experience. Education Bachelor’s degree required, preferably in social work or related field.
