Mileage reimbursed! Pay: $34.13/hour Schedule: Mon-Fri, 8:00am-5:00pm Overview We are seeking a self-motivated, detail-oriented, and tech-savvy Healthcare Consultant I/Field Case Management Coordinator to join a Case Management team supporting members across the West Hialeah region. This role combines remote case management with in-person visits to members in their homes, assisted living facilities, and nursing homes. Training is conducted remotely via Microsoft Teams for approximately 4–6 weeks. Key Responsibilities Assess, plan, implement, and coordinate case management activities to support member wellness Manage a caseload including supportive and medically complex members Develop proactive care plans to improve short- and long-term health outcomes Coordinate care and services based on member needs, including: Prior authorizations Coordination with primary care providers (PCPs) and skilled providers Medication reviews Condition management education and support Community resources and support services Communicate clearly and professionally with members and care teams (phone and in-person) #J-18808-Ljbffr A-Line Staffing Solutions
