Description

Under the general supervision of the Support Services Director, the Nurse-Support Services is responsible for ensuring that eligible individuals receive quality health and medical services, through either Medicaid Waiver or locally-funded service provision.

Qualifications

Education/Experience Minimum of 18 months full-time nursing experience (or part-time equivalent), part of which includes providing nursing services to individuals with disabilities

Credential/License Ohio licensed registered nurse. Must obtain and maintain First Aid and CPR certification. American Red Cross Instructor Adult and Pediatric First Aid/CPR/AED certification

Other Knowledge of common health conditions and treatment. Must have reliable transportation. Must maintain a valid State of Ohio Driver's License if operating a personal vehicle for official agency business.

Essential Functions

Evaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to:

1. Completes Nursing Quality Assurance Assessments and all associated follow-up in accordance with rule. (30%)
2. Provides oversight, monitoring, and guidance regarding health and medical services in residential and day service locations to improve the quality of service delivery by providers in those settings, including certified Medicaid Waiver providers. (14%)
3. Provides consultation and information for Investigative Agents in the course of conducting MUI Investigations. (2%)
4. Identifies potential needs and barriers to quality service delivery and makes recommendations to SSA's, providers and other team members for individual's served. (5%)
5. Assists the Support Services Director with developing policy and procedures related to nursing (2%)
6. Participates in stakeholder meetings and work groups and collaborates with county boards, providers and DODD representatives to improve service delivery. (2%)
7. Conducts health-related trainings for certified Medicaid Waiver providers and county board staff, in order to support providers in knowledge and ability to deliver services. (25%)
8. Assists with gathering or providing information for Service Consultants in advance of assessment of medical needs, including those related to Medicaid Services. (5%)
9. Maintains records related to the health and medical needs of individuals served (5%)
10. Reviews and monitors health status of individuals served through documentation and communication with team members and makes recommendations to teams regarding services, care, planning and follow-up (10%)
11. Working Knowledge of all DODD Medicaid Waiver rules and regulations
12. Other duties as assigned.

To successfully perform this job, the following training, knowledge, skills and abilities are required.

Required Training: All UCBDD employees shall be required to complete all general mandatory training. These include, but are not limited to:

• Blood-borne pathogens, emergency communications, hazardous communications, slips/trips/falls, personal protective equipment, sexual harassment, drug free work place, violence in the workplace, MUI and fire prevention.
• Must obtain and maintain Board approved behavior support and crisis intervention training.

Knowledge: Proficient with computers and applicable software, ability to use general office equipment. Working knowledge and understanding of behavior support, individualized service plans, medical, psychosocial, orthopedic and psychiatric aspect of disability. Working knowledge of client rights, federal, state, UCBDD programs and HIPAA. Knowledge of applicable statutes, regulatory standards and reporting authorities

Language/Mathematical Skills: Ability to communicate effectively, both orally and in writing. Ability to perform basic math skills.

Reasoning Ability: Must be skilled in group facilitation, conflict resolution, crisis intervention and working through complex problem situations.

Other: Must possess a positive customer service attitude and image to all stakeholder groups. Must be able to work collaboratively and effectively with other agencies, professionals, staff, families and the public. Good leadership and organizational skills. Ability to keep accurate records. Must be able to tolerate the high demands of the position. The employee may be exposed to adverse weather conditions while driving and may be exposed to blood-borne pathogens, communicable diseases, potentially infectious materials and/or aggressive behavior.

Maintaining confidentiality is required. The employee is to report suspected or actual abuse/neglect, to follow the chain of command and to work effectively and cooperatively with others inside/outside the agency. Regular and predictable attendance is expected.

Preserving a valid certification, licensure or registration is expected for those positions that require such in order to continue employment. It is essential for the employee to maintain all training and inservices required by the position.
