Case Management Nurse

Job Description Telenett is seeking a dedicated and compassionate Case Management Nurse to join our team in Albany, GA. As a Case Management Nurse, you will be responsible for coordinating and managing the care of patients with complex medical needs. Responsibilities Collaborate with healthcare team to develop and implement individualized care plans for patients with complex medical needs Monitor patient progress and make necessary adjustments to care plans Educate patients and their families on treatment plans and self‑care techniques Coordinate with insurance companies and other healthcare providers to ensure proper coverage and continuity of care Conduct regular assessments of patient needs and provide appropriate interventions Advocate for patients and ensure they receive quality and timely care Document all patient interactions and maintain accurate records Stay up‑to‑date on medical advancements and best practices in case management Participate in team meetings and case conferences to discuss patient care and progress Adhere to all HIPAA and patient confidentiality guidelines Requirements Valid RN license in the state of Georgia Minimum of 2 years of experience in case management or a related field Strong communication and interpersonal skills Ability to work independently and as part of a team Excellent organizational and time‑management skills Knowledge of medical terminology and procedures Familiarity with insurance processes and regulations Compassionate and empathetic attitude towards patients Ability to handle sensitive and confidential information with professionalism and discretion If you are a dedicated and caring individual with a passion for helping patients with complex medical needs, we encourage you to apply for this exciting opportunity at Telenett in Albany, GA.
