Responsibilities Develop and implement patient‑specific therapeutic interventions to improve quality of life. Work collaboratively with physical therapy assistants and a multidisciplinary team. Assess and treat patients of all ages and abilities in an acute‑care rehabilitation setting. Interpret data and document progress within the EPIC electronic charting system. Coordinate and communicate with various disciplines or services involved in patient care. Facilitate discharge planning and establish goals during evaluation and throughout the episode of care. Advocate for patient self‑determination and optimize resource utilization to achieve the highest level of independence. Provide mentorship and orientation support to new team members. Perform additional duties as assigned. Qualifications Bachelor’s degree required; Doctor of Physical Therapy preferred. New graduates are encouraged to apply.
