Nurse Practitioner for In-home Visits

We’re hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on a person’s overall health and well‑being, identifying potential barriers to care. Review medication and medical history, and perform a physical evaluation in a member’s home. Provide a pre‑populated, patient‑specific form for each evaluation and do not prescribe medicine, order lab tests, or alter existing treatment regimens. Compensation and Schedule Earn up to $2,500 to $4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment). Your per‑visit rate can increase up to 135% with our Market Multiplier. Additional pay‑per‑test compensation for visits where the member qualifies for diagnostic or preventive tests. Availability incentives based on weekly availability. Schedule: Monday‑Sunday shifts; no nights or weekends required. Short‑term or long‑term basis. Travel compensation (airfare, hotel, rental car) may be provided depending on eligibility. Qualifications Board‑certified nurse practitioner. State license required. Medicaid license or willingness to enroll (with our support) strongly preferred. Independent contractor, 1099 status. Ability to work flexibly and adhere to a pre‑scheduled visit schedule.
